Consent Form Details
- Consent for Participation: This letter serves to give permission for my/our child to participate in the activity/trip/camp. I/We acknowledge that my/our child is expected to abide by RAS school rules and always conform to RAS teachers’/staff’s instructions to ensure a safe activity/trip/camp both within and beyond the school vicinity.
- Reading and Consent: I/We have read all of the information contained in this letter in relation to the activity/trip/camp, and hereby freely and voluntarily give consent for my/our child to participate in the Raffles American School Camp.
- Medical Authorization: In the event of an accident or illness, I/We fully authorize RAS staff to obtain or administer any medical assistance or treatment that my/our child may reasonably require, including contacting a doctor.
- Cost Responsibility: I/We shall be personally responsible for all costs incurred by RAS in obtaining such medical assistance or treatment (including transportation costs) and undertake to reimburse RAS the full amount.
- Child's Fitness Declaration: I/We have provided RAS with all pertinent information concerning my/our child's fitness, affirming that they are medically, physically, and mentally fit for participation.
- Right of Refusal: I/We acknowledge that RAS reserves the right to accept or decline my/our child's participation if they are deemed to be causing an undue disturbance or posing a danger.
- Data Processing Consent: I/We consent to RAS storing and processing the Personal Data provided in this letter for the purposes described herein and for disclosure as required by law.
- Acknowledgment of Risks: I/We recognize that participation in the activity/trip/camp may inherently entail unavoidable risks, integral to the adventure and social development of students.
- Force Majeure Limitation: I/We acknowledge that RAS, its officials, and staff are not responsible for force majeure events beyond their control (e.g., Acts of God, war, natural disasters).
- Acceptance of Personal Risks: I/We willingly accept all inherent risks associated with participation, including potential personal injury, loss of life, or property damage.
- Release of Liability: In consideration of participation, I/We irrevocably release RAS and its representatives from any liabilities, claims, or demands.
- Indemnification: I/We commit to indemnify and hold harmless RAS, along with its directors, employees, agents, and volunteers, against any actions or omissions carried out in accordance with provided directions.
- Dorm Allocation Policy: I/We acknowledge that only RAS may change dorm allocations after final applications have been submitted, with all related requests due before application closure (for boarding students).
- Placement Decision: I/We understand that RAS will make the final decision regarding my/our child's placement in a class based on age and English proficiency testing procedures.
- Refund Policy: I/We understand that RAS practices a no-refund policy, except in the event of a denial of entry or registration by Malaysian authorities, in which case a full refund (100%) will be processed within 30 days upon submission of official proof.
- Responsibility for Property: I/We acknowledge that safeguarding the student’s property is primarily the student’s responsibility, although RAS will make reasonable efforts to locate lost items.
- Intellectual Property Rights: I/We understand that all intellectual property (including photos, videos, trademarks, and other materials) related to the camp is owned by RAS, and hereby waive any rights thereto.
- Day Students’ Supervision: I/We understand that day students must have a parent or guardian responsible for their care after camp hours, as RAS is not liable for their supervision or safety outside official hours.
- Electronic Agreement: By checking the required box on the registration form, I/We acknowledge that this constitutes our legally binding electronic signature under Malaysia’s Digital Signature Act 1997.
- Governing Law: This Consent Form shall be governed by the laws of Malaysia. In the event of a conflict, the English version shall prevail.

- Injuries from sports or recreational activities.
- Exposure to outdoor elements, nature, or weather conditions.
- Inexperience or unfamiliarity with activities.
- Unfamiliarity with location or facilities.
- Faulty equipment or inadequate instruction.
- Complications from weather or environmental factors.
- Limited healthcare facilities or assistance.
- Allergic reactions, illnesses, or negligence.
Recommended Precautions:
- Check local weather before departure.
- Bring appropriate clothing, footwear, and protective gear.
- Secure valuables and avoid carrying unnecessary items.
- Inform RAS of any medical conditions.
- Carry necessary medications or emergency kits.
